Demographics: 23 year old; Kate
Symptoms: Hair loss, skin rash, and fatigue. She went to the ER after having severe tremors and muscle spasms.
History: She is recently recovering from a 3-week infection and has just stopped taking antibiotics. She is allergic to eggs, peanuts and fish and she does not eat pork for religious reasons.
24hr Diet History:
Breakfast: Pancakes with butter and syrup and a small glass of orange juice
Lunch: Hamburger and French fries with a large Coca-Cola
Dinner: Fried chicken, mashed potatoes and gravy, and corn with a glass of sweet tea
Findings: Patient present with episodes of convulsions
Test Results: None that were noteworthy
